Holes in Leaves - Flowers Damaged - Visible Insects Described

Leaves have holes in them, and the flowers are damaged. They may have holes in them. Also, there are visible insects on the plant. These insects are about 1/4 inch in length. They may be striped, banded or spotted, and they are black and either yellow or yellowish-green in color. If your African Violet has this combination of symptoms, it may be suffering from the following:
